Here’s a question I got from a guy who’s interested in Cock and Ball Torture, better known as CBT. While it’s not my bag, I thought it was interesting enough to answer. Check it out:

I am very interested in getting into CBT but I am worried about potential health problems, primarily fertility. Unfortunately, googling CBT and health is only giving me results for Cognitive Behavioural Therapy which, while interesting, is not what I am looking for. If you have any personal knowledge, I would greatly appreciate it. Thanks!

To start, CBT isn’t my thing, and I’m no medical expert, but from what I’ve seen from friends of mine, I assume that CBT — Cock and Ball Torture — is fairly safe in the right hands (yes, that made me laugh). As long as you aren’t too brutal with Heavy D and the Boyz, you should be fine. Keep in mind to start off light, and know your limits!

Other things to be aware of is that ball crushing is a LOT different than ball stomping or kicking, which is STILL different than ball stretching. All men are different, and their testicles can take various kinds of punishment in many ways. But erections depend on blood flow, so any trauma brought on by unsafe CBT play could result in erectile dysfunction, and those tiny blue pills may not help. And what can rupture one man’s bollocks may just be a warm-up to another. Yes, mate, testicles CAN rupture, but it takes a lot of pressure to do it — or just the right twist.

Twist and Shout

Speaking of twisting, I’d advise against actually twisting them, as testicular torsion is a very real thing. In case you’re wondering what the fuck that is, testicular torsion happens when a testicle rotates, and twists the spermatic cord that brings blood to the scrotum. And if that sounds painful, then you’re absolutely correct.

If you have a testicular torsion, chances are you’ll know it. You’ll feel a sudden, possibly severe pain in your scrotum and one of your testicles. The pain might increase and decrease but generally won’t go away completely. Now, testicular torsion is a serious thing, and you’d need to hit the ER, as it needs to be diagnosed. The nice thing is, however, is that it’s treatable, and after it’s corrected by a medical professional, it’ll get sorted within a few weeks.

To continue, testicular torsion can cause a lot of unwanted damage — like actually losing one. And if you plan to do any tying, don’t cut off your blood flow for too long for this same reason. Just be fucking careful, is what I’m trying to say.

What About CBT and Fertility?

As far as overall fertility goes, it’s a relatively unknown thing. This is mainly because there aren’t a lot of studies that deal with the potential negative effects that CBT can bring to a man’s fertility — and yes, this also includes ball stretching. It’s still unknown whether these things can lower a man’s fertility or make him completely infertile.

There are people who claim that CBT has no effect on fertility and that men can enjoy it without any worries for the quality of their sperm. Others claim the opposite, saying that CBT definitely has a negative effect and that it can cause infertility. So the evidence that’s present right now is a bit all over the place, because there’s not enough of it to draw a proper conclusion. Personally, I’m on Team Negative Effect. Got a T-shirt and everything.

The only advice I can give you, or any bloke who wants to absolutely prevent any negative effects on their fertility is to avoid any form of CBT activities at all. This is the only way to make sure that cock and ball torture, including ball stretching, will not affect your fertility.

But that’s no fun, right? Look, as long as you start out light it’s unlikely — but theoretically possible — that there would be any problems. I’d also consider having your sperm frozen, just in case. Like I said, I’m no medical expert, so you might want to ask a doctor about any concerns.

With this said, here are a few things to keep in mind if you’re considering CBT:

  • Go slowly at first, and start off light. The intensity and sensation can gradually be increased.
  • Any bondage should be something that can be easily removed.
  • Tools like safety scissors should be on hand and easily accessible for quick release.
  • Clamps, weights, or anything that binds or puts pressure on the genitals should NEVER be left on for too long. When you start to notice redness and/or bruising, that’s usually the indicator.
  • You should NEVER be left unsupervised during CBT.
  • More extensive play, like sounding or piercing, requires a LOT of knowledge and expertise — like risk assessment and sanitary precautions — before attempting it. If your partner is brand new at CBT, I seriously advise holding off until they know what the fuck they’re doing.

In Conclusion

Like I’ve mentioned three times already, be sure to start off light. In addition, you should start by discussing your roles, and you should definitely know your limits ahead of time. I’m sure you know (at least you should know) that everything must be consensual and negotiated beforehand, which also includes having a safeword in place.

Another thing I feel I need to mention (because some of you are just fucking stupid) is that cock and ball torture should NOT be used as an attempt to become infertile, so don’t use it as such. There is a big chance that it won’t work (read: probably WON’T work) so don’t rely on it. Those of who don’t want children should either use birth control, or get a motherfucking vasectomy. You’ve been warned.
